Roofing enquiries are easier to route when the request identifies whether the property is a detached home, townhouse, strata building, commercial property or another type of site. If you know whether the roof is asphalt shingle, metal or flat membrane, include that too. If not, a simple description such as steep sloped roof or low-slope commercial roof is still useful.
For leaks, describe the interior location and whether the moisture appears during every rain or only under particular wind or storm conditions. Safe photographs can provide context, but nobody should climb onto a wet, icy or steep roof to obtain them. The purpose of the form is to start the conversation, not replace an on-site assessment when the cause is uncertain.
